One thing people rarely talk about in software development is how important the boring foundation decisions are like choosing the right database and infrastructure. And in healthcare, those decisions matter a lot MORE. We’re currently deep in the foundation stage of building ilarna: setting up servers, databases, and all the underlying infrastructure the product will run on. In healthcare, this matters even more because of patient confidentiality, compliance, and interoperability standards. We decided to build on Microsoft Azure. The main reason is that Azure is built for healthcare workloads. It’s designed to handle Protected Health Information (PHI) and supports global healthcare standards like FHIR (Fast Healthcare Interoperability Resources) and DICOM out of the box. That means a lot of compliance and certification work is already handled at the infrastructure level. If we went with a more generic cloud setup, we’d likely have to register, certify, and integrate many of these standards separately, which usually means more time, cost, and more risk. It’s one of those decisions that seems invisible but has a massive impact on compliance, partnerships, and how easy it is to scale later on.
