Great reflection from Dr Phil McElnay, CEO of MedAll on the future of global medical education.
Yesterday, during a panel hosted by the Global Alliance for Medical Education, my wonderful friend and colleague Kristen Dascoli posed a question that cut straight to my heart. "How do we address the needs of learners in resource-constrained environments? Are there opportunities to leverage 'Communities of Practice' to bridge gaps?" For decades, the traditional global medical education has operated with a massive blind spot: the assumption of knowledge asymmetry. We’ve treated high-resource hospitals as the "creators" of wisdom, and low-resource clinics as the "charity cases" that simply need our broadcasted lectures. But when you strip away the million-dollar diagnostic machines and unlimited budgets, what are you left with? Pure clinical reasoning. The truth is, clinicians in resource-constrained environments are the world's foremost experts in frugal innovation, resilience, and diagnostic agility. Today, high-resource healthcare systems - many of which are buckling under severe staffing crises and burnout - desperately need to learn from these clinicians, not just lecture to them. And needs in each country vary - sometimes wildly. This is exactly why Communities of Practice are so vital. We are passionate as a team at MedAll not to see education as a one-way street for "Information Charity" - English content translated into a few languages with no cultural competency. We are much more interested building the digital infrastructure for Knowledge Equity. When we connect clinicians across borders in low-bandwidth, highly collaborative spaces, we shift outcomes and we actually allow for reverse innovation. A workaround developed in a rural clinic can inform a crisis-response protocol in a major urban ER. We have to stop treating global education as a top-down broadcast and start building bi-directional bridges. A massive thank you to Kristen for the brilliant provocation, and to the wonderful team at GAME for hosting such a necessary conversation about the future of our profession. TechForGood published my latest piece on what Claude Science actually signals for biotech AI. In short, when a frontier lab establishes scientific reasoning as a core capability, standalone AI tools lacking proprietary data, commercial traction, or a clear role within a larger system become vulnerable. This is becoming a consensus view. The less comfortable part is that many investors are responding by backing wet labs as a physical barrier. I think this trade is more crowded and less durable than it seems. Remote experimentation and automated foundries are turning lab infrastructure into a commodity: rentable capacity, not a moat. Where does durability live? Downstream in messy, human-mediated systems that AI compresses last, such as clinical development, regulatory pathways, market access, and patient adoption. That's where proprietary data accumulates and regulatory clearances compound. It's also where workflow position is genuinely hard to displace. This is the lens we use at Nina Capital, and I lay out the full argument in the article.
