Healthcare buyers stopped reading feature lists. They started drawing workflows. Five years ago, a vendor pitch was a slide of capabilities. Today, the slide that closes deals is a sequence diagram. Order placed here. Result returned there. Clinician signs off in the chart. Bill drops automatically. If your product cannot draw itself onto that diagram, it does not exist to the buyer. This is the quiet shift that most healthtech startups have caught up to. Features are easy to demo. Workflows are what hospitals actually buy. A workflow sale answers questions a feature sale never does. Where does the data come from? Where does it go? Who clicks what, and when. What happens if the network drops? What does the audit log look like at midnight? The product still matters. But the product is now scenery. The workflow is the script. This is why integration-native vendors are pulling ahead. Their pitch is already a flowchart. Their competitors are still selling screenshots. If your sales deck has more product shots than workflow diagrams, you are pitching to a buyer that no longer exists. The new moat in healthcare isn't AI. It's connectivity. Every healthtech pitch this year claims an AI advantage. Better model. Better fine-tune. Better agent. Hospitals nod. Then they ask the only question that decides the deal. "How does this get into our EMR?" That single question erases most of the AI differentiation in the room. Because models are converging fast. Open weights. Cheap inference. The same foundation layer will be available to every competitor by next quarter. What is not converging is the path into Epic, Cerner Corporation, eClinicalWorks, athenahealth, MEDITECH, and the next forty smaller systems a real deployment touches. That path is brutal. SMART on FHIR launch contexts. HL7 v2 feeds. Interface engine politics. Site-specific Implementation Guides. Encounter binding. MPI quirks. Write-back permissions that vary by department. None of this shows up in a model leaderboard. All of it shows up in a procurement gate. The healthtech companies pulling ahead right now are not the ones with the best AI. They are the ones who can deploy that AI inside a clinician's existing workflow without forcing IT to babysit a new integration. Connectivity is the layer competitors can't copy in a sprint. Every healthtech startup eventually wakes up with two engineering teams. One they hired. One, they did not. The team they hired builds the product. The team they did not hire keeps eight hospital integrations alive. It does not start that way. The first integration feels like a one-time cost. A senior engineer disappears for a quarter, ships it, and moves on. Then the second site goes live. Then a third. Then, Hospital A upgrades its Epic version on a Saturday, and a feed starts dropping silently on Monday. Now someone has to own that. Then someone has to own the on-call. Then someone has to own the version matrix. Then someone has to own the analyst tickets that arrive in Spanish at 11 PM because a downstream system stopped accepting your CDA. Nobody planned this. It just accumulates. Most founders do not see it because it does not show up as a line item. It shows up as a velocity loss. Roadmap items slip. Senior engineers stop touching the product. Hiring plans tilt toward "integration engineers" — a role that did not exist in the original org chart. By Series B, a meaningful share of your engineering payroll is not building the thing customers pay for. It is keeping the connection to customers alive. That is not a hiring problem. That is an architecture decision you made before you knew it was a decision.
