When you've been part of building something from the beginning, moments like this mean a little more. The recent FDA approval of the on-body injector with Sanofi has taken years of innovation, trust, and teamwork. Seeing our enFuse® technology reach this milestone is incredibly rewarding as it’s evolved from an idea into a commercial reality with this second approval from the FDA. This achievement with Sanofi reflects what can happen through strong partnership, thoughtful collaboration, and a shared focus on patients. I'm proud of Enable's journey so far with Mike Hooven leadership and Dave Stefanchik technical expertise, and I'm excited to see the impact this will have for patients and caregivers. History was made today in oncology. Our on-body injector administering subcutaneous formulation has become the first device of its kind FDA approved to administer an oncology therapy subcutaneously – specifically for patients with multiple myeloma across numerous combinations and lines of therapy. What could this mean in practice? For multiple myeloma patients: a small, hidden, retractable needle designed with comfort in mind. Treatment delivered in minutes through a wearable device. For nurses: hands-free delivery. The potential to reduce physical burden and needlestick injury risk, while also providing time to focus on patient care. For pharmacists: streamlined preparation and supplies, in addition to a fixed-dose formulation that may reduce the risk of medication errors. For industry: further evidence of the potential of patient-and-provider centric innovation to improve large-volume subcutaneous drug delivery. The collaboration between Sanofi, Blackstone, and Enable Injections, Inc. was announced in 2022. When I started at Enable at the end of that year, this was the story that sold me. A 3 mL subcutaneous delivery ceiling exists. It governs one type of delivery. Most of the industry has been applying it to both. If you're delivering the full volume all at once in a few seconds, 2-3 mL is a real, well-established limit. If you're delivering over minutes via an infusion pump or on-body injector, that ceiling doesn't apply. With time, the volume tolerance has been tested and approved over 100 mL. Same anatomical space. Two completely different volume limits. And the industry routinely conflates them. We aggregated data across four independent surveys of 378 pharmaceutical professionals spanning CMC, formulation, clinical development, medical affairs, commercial, and R&D and portfolio decision-makers: 65% identified ≤3 mL as the upper limit for subcutaneous bolus delivery without a permeation enhancer. 62% identified that >3 mL is feasible for subcutaneous infusion delivery without a permeation enhancer. In other words, more than 1 in 3 experts (38%) still believe subcutaneous infusion is capped at 3 mL. It isn't Now compare that to what's already on the market. Several biologics delivered well above 3 mL subcutaneously, without a permeation enhancer, have been approved and used in clinical practice for over two decades. Volumes across these approved products range from 3.5 mL to >100 mL, delivered via syringe pumps and on-body injectors. These are approved, prescribed, and used frequently in clinical practice today. Conflating the bolus ceiling with the infusion ceiling is one of the most consequential rounding errors in subcutaneous drug development. It eliminates options before a development team has even drawn the first formulation. We've personally heard of projects coming to an end because of a belief in this myth. Promising candidates that could have reached patients, scrapped because someone in the room concluded the volume couldn't be made to work. The science was there. The clinical rationale was there. The delivery options were there. The misconception was the only thing standing in the way. That's the cost of a rounding error nobody ever stops to question. #Biotech #Biologics #DrugDevelopment #Formulation #Subcutaneous

Let me show you the burden most people don't see. During the early years of my pharmacy career, I worked in sterile compounding in the hospital pharmacy. Counting tablets. IV admixtures. Standard small-volume injections. Long shifts on your feet. What I didn't face in the hospital pharmacy: large-volume subcutaneous biologics. Now they're a routine part of oncology, immunology, and rare disease care. And the preparation burden is accelerating. Here's what current pharmacists are dealing with: -Repetitive strain injury risk from viscous, high-force manual syringe preparations -Long preparation times and drug waste from cancellations -Multi-step protocols in cramped sterile compounding spaces -Needlestick injury risk on every manual preparation Here's what the data showed: 100% said on-body injectors are easy to prepare and easy to learn. 87% preferred on-body preparation over syringes for three reasons: → Less time required → Less effort required → Flexible preparation location What this solves: 97% said it would reduce burden 100% said it would improve efficiency 90% said it would reduce preparation errors 100% said it would eliminate needlestick injuries The next generation of large-volume subcutaneous drugs should be designed with this evidence in hand.
