A step in that direction is rolling out in product today! EvidenceGrade reflects our attempt to adapt the GRADE criteria (gold standard approach used by Cochrane et al) to the unique constraints of a real-time system dealing with messy queries, etc. It’s not perfect — feedback is very welcome — but our internal testers really liked it a lot and I hope the community does as well. This feature was designed and built with Travis Zack, MD, PhD, Eric Lehman, Evan Hernandez, and other incredible engineers on the OpenEvidence team. Ania Bilski, MD liked thisAnia Bilski, MD liked thisFor the past two weeks, our independent team of statisticians, AI evaluation experts, clinical AI researchers, and clinicians was given a unique opportunity to test one question: “How well do different AI tools answer user questions on the OpenEvidence (OE) platform?” We were given access to 620 benchmark questions sampled from real queries submitted to OE, and 149 practicing physicians across 36 states made blinded head-to-head comparisons between answers from three frontier general-purpose models (Claude Opus 4.8, Gemini 3.1 Pro, and GPT5.5) and OE’s specialized clinical AI tool. Graders were matched to questions that matched their specialty to maximize evaluation accuracy. Today we are sharing the results on arXiv and the benchmark dataset on HuggingFace. Following our prespecified statistical analysis, OE scored highest on all five dimensions of accuracy, clinical utility, source quality, verifiability, and completeness. On the primary endpoint of win differences (a model’s win rate minus its loss rate in head-to-head comparisons), OE led by 25–39 percentage points all these dimensions (p < 0.001). Results were consistent across our *many* sensitivity analyses. So, how do these results square with recent work reporting general-purpose LLMs outperform OE? We think both findings can be true: when a specialized tool is evaluated in a setting outside of its intended workflow, it may underperform. But the more relevant question for AI application developers is whether their tool is delivering value in its intended use case for its target users. On that measure, our findings are positive. At least for now, the hard work of engineering and customizing an AI system can still pay off, delivering meaningful performance gains to its target users. When I was a medical student, Dr. Joel Katz, then the internal medicine residency program director at Brigham and Women's Hospital, asked me what I thought the mission of his hospital was. I said something earnest about providing great patient care. He told me the mission was to be the best training ground in the country for the next generation of physicians. That answer threw me at the time, but has organized my thinking ever since. He was not saying patients come second. He was saying that if you build the finest possible place to train and support physicians, you do not have to choose. A well-trained, well-supported physician is the mechanism by which good care happens at all. I wrote about that conversation, and what it means for how we build technology for physicians, in my first essay for OpenEvidence's new blog. Now, clinicians asking a question at the point of care can receive not only an evidence-based answer in text, but a peer-reviewed surgical video, performed by an established surgeon, shown exactly when and where it's needed. "Video gives insight that text alone cannot—especially in surgery. Over the last decade, JOMI has worked with surgeons to publish a large and growing library of peer-reviewed surgical videos that offer a uniquely immersive learning experience. We are excited to collaborate with OpenEvidence to bring this content to clinicians when and where it has the greatest potential for impact: embedded directly in answers to clinical questions," said Nikita Bernstein, CEO and Co-Founder of JOMI. OpenEvidence has trained a model on JOMI's peer reviewed surgical videos that highlights the relevant video segment pertinent to the medical question, rather than an hour-long recording. 🔬 Stanford Healthcare Innovation Lab runs over 3,500 immunohistochemistry cases every single month. Back in 2005, that number was 700. The workload has quintupled and the workforce hasn't kept up. You know what that means in practice: more pressure, more variability, more room for error. Researchers at the University of Pennsylvania and Stanford University built iSight to help. It's a multi-task AI model for automated IHC staining assessment, trained on HPA10M, a freshly curated dataset of over 10 million IHC images across 45 normal tissue types and 20 cancer types. Nothing like it has existed at this scale before. 🧫 Applications and Insights 1️⃣ Three staining tasks, one model iSight predicts intensity, localisation, and quantity simultaneously, hitting 85.5% accuracy for location, 76.6% for intensity, and 75.7% for quantity, beating fine-tuned PLIP and CONCH by 2.5–10.2%. 2️⃣ Calibration you can actually trust Expected calibration errors sat between 0.015 and 0.041. The confidence scores reflect real accuracy, which matters enormously when a clinician is deciding whether to act on a prediction. 3️⃣ It makes pathologists more consistent Eight pathologists evaluated 200 images before and after seeing iSight's suggestions. Inter-rater agreement jumped from Cohen's κ 0.63 to 0.70.
