Health Tech apps fail for one simple reason. Patients don’t decide what gets used in healthcare. Founders assume patients adopt healthcare apps the same way they adopt consumer apps. They don’t. Consumer apps spread through convenience. Healthcare tools spread through trust. Take something simple like strep testing in Urgent Care. Primary care physicians see the same conditions repeatedly. If an app helps streamline that workflow, adoption happens quickly. Now take the dynamic changes with specialty care. A patient dealing with a heart condition isn’t experimenting with random apps. They’re following their physician’s guidance. And that distinction matters. Patients may use the technology. But physicians drive the adoption. When a clinician recommends a tool, patients trust it. Without that even the best technology struggles. This is where many AI health tech companies may miscalculate. They design products assuming the patient is the decision maker. And in reality, adoption is usually mediated. Which means if you want adoption… You have to earn physician trust first.

High level area I find interesting - where in healthcare can we create “ground truth” where currently a proxy is being used? EHR documentation is a proxy for what actually happened during the visit, can a scribe provide ground truth? Capturing random point in time biomarkers are a proxy for the ground truth of what's going on with a patient during the course of their lives. Instead of point in time A1c capture for risk adjustment, can we use CGMs to monitor what's happening regularly? Claims data is used as a proxy for how patients are getting care across the health system, can an AI care navigation company create a more ground truth map?

For years, PMS migrations and insurance verification have been a black hole in dentistry. Everyone feels something is breaking once they migrate to a new PMS. No one can see exactly where. And decisions get made on anecdotes, many discussed during conventions, gut feel, and social proof (yes, "I feel good because I use the same shinny object as everyone else"; pure nonsense). India, with its huge patient population and ever increasing digital infrastructure presents an unique opportunity for Digital Health solutions to make an impact. However, the fragmented (mostly private) system presents a challenge on establishing recurring reimbursement pathways. Clinician capital in India today flows mostly through hospital groups or angel syndicates — not through independent funds. That’s very different from models in the west.
