Sam G. さんがリアクションしましたSam G. さんがリアクションしましたBREAKING: MASSIVE DISRUPTION in to RPM in CMS 2027 Medicare Physician Fee Schedule Proposed Rule. Amongst other significant changes, CMS is proposing that staff furnishing RPM and RTM services must be directly employed by the billing practice. This will massively disrupt a huge amount of RPM being serviced to date. Preventive care is notoriously difficult to scale at the CMS reimbursement rates, there's a reason so much of it is outsourced. The funny thing is that the OIG reason is to prevent fraud. in my experience all the fraud happens when new clinical entities are set up so that a RPM company can bill themselves. People who want to milk the system want to collect 100% of the reimbursement, not share with a PCP/cardiologist. They also don't want to have to answer to their doctor client who will demand complete compliance and quality. The best RPM companies are the ones doing it on behalf on small to medium sized doctors.

Healthcare environments extend beyond standard CRUD architectures. EHR systems, HL7/FHIR integrations, legacy imaging infrastructure, cloud-native services and AI-assisted clinical tooling form interconnected systems within regulated environments, expanding both operational dependency and attack surface. At this scale, CVSS scores, static findings, manual triage and spreadsheets alone don’t scale, especially in healthcare provider environments where security decisions impact clinical continuity, uptime and patient trust. Modern healthcare AppSec requires: ~ Clear visibility into applications processing protected health information (PHI). ~ Risk reporting aligned to regulatory frameworks by design (HIPAA, HITRUST). ~ Evidence of exploitability, not just vulnerability volume. ~ Prioritization based on runtime execution and reachability. Legacy EMR modernization: How we preserved clinical workflows while updating infrastructure The Client: Multi-specialty therapy practice management platform The Challenge: 20-year-old system needed modernization without disrupting 1000+ practices The Timeline: 4 months (vs 12-month traditional estimate) The inherited complexity: Appointment scheduling workflow built in 2005. Each step existed for specific reasons: Insurance validation (regulatory) Provider availability (business logic) Room assignment (operational) Patient history review (clinical workflow) Billing code verification (revenue cycle) Why internal teams struggled: They tried "simplifying" three times over 5 years. Each time, removing "redundant" steps created downstream problems. So they stopped trying. Our approach: Week 1: Shadow front desk staff at 3 practices ➡️ Understand actual workflow vs designed workflow Week 2: Map what data systems already have vs what requires user input ➡️ Identify automation opportunities Week 3: Build intelligent backend that handles validations automatically ➡️ Preserve workflow but automate constraints Week 4: Design progressive interface based on booking complexity ➡️ Simple cases fast, complex cases complete The transformation: Simple appointments (70% of volume): System auto-suggests provider based on patient history + availability System auto-assigns room based on provider + equipment needs System pre-fills billing based on insurance + visit type Staff confirms and adds notes Complex appointments (30% of volume): Multiple providers needed Special equipment required Insurance requires manual verification System escalates to detailed workflow ⭐ Results: Time: 6-8 minutes → 90 seconds average Errors: 12% → 3% Satisfaction: 89% staff approval in testing Adoption: Rolling out to 50 practices, full deployment Q1 2026 The key insight: We didn't simplify the workflow. We automated the constraints that 2005 technology couldn't handle. 2005 reality: Manual insurance lookups Static provider schedules Separate room booking systems User-entered billing codes 2025 capability: Real-time insurance eligibility APIs Dynamic provider availability across systems Automated room matching Intelligent billing code prediction Our role: Not teaching the client about therapy practice workflows (they knew better than us). Questioning which 2005 constraints still applied in 2025. Then redesigning workflows assuming modern platform capabilities. Modernizing legacy healthcare systems without disrupting operations.
