Steve Tremitiere shared thisI SAW 100,000 PEOPLE SMILING! Well, maybe I didn’t see every single smile but they were all there and I felt each one. Talk about a REAL walk - I did the Bolder Boulder 10k Monday for the first time - one of the biggest and best races in the country with about 55,000 runners / walkers - and I was deeply moved by how happy EVERYONE was. The participants, the volunteers, the spectators, law enforcement…everyone was smiling. I can’t remember the last time I experienced that. With so much anxiety, uncertainty and stress creating massive mental health challenges individually and collectively, the opportunity to laugh and smile and enjoy other humans was incredible. I have bad knees so I had to walk, but I’m glad I did - it gave me time to take everything in - the mass of humanity, the bands playing in people’s yards, belly dancers and drumming groups (it is Boulder!), the kids spraying hoses as people walked by, tequila shots being served by water gun, etc… No brilliant insights - just tremendous gratitude and the knowledge that community and human connection is the foundation of mental health!! BTW the race ends at the stadium so the vast majority of the people in the stands were race participants who stayed to cheer on others! THE REAL Mental Health Foundation

I couldn’t do a walk this month because I was busy having an incredible weekend with my amazing family, including birthday parties, coaching grandchild softball games and just enjoying our incredible Colorado Crew. A day doesn’t pass when I don’t speak with someone who is a part of the incredibly important work all of us are doing to raise awareness around the importance of mental health support. The movie about the Boston Globe reporters who exposed the child sex abuse scandal in the Catholic Church. I was abused in a church. So yes, it brought up a lot. There’s a scene where the survivors are being interviewed and the crimes are finally coming into the open. In that moment Apollo reached over and held my hand. A simple gesture. Kind. Emotionally aware. I sat there in the dark feeling something shift. I grew up in a very different kind of home. That moment would never have happened between me and my father. It felt like a lineage changing in real time. Leaving a 30 year finance career in my 50’s to do this work has been a wild ride. It’s been a blessing to find purpose. For 30 years my work ran on fear. I believed if I acquired enough I would finally be safe. On the other hand, this new path has surfaced deep fears and questions of self-worth. With my kids out of the house, I could have built a life oriented around less stress. Instead I get to deal with as much internal material as ever. But there is a difference now. The career and the addiction were both ways of running. Today through a daily spiritual practice, I am dedicated to something deeper. Not just facing what I ran from. Experiencing that I am good and whole, even in the middle of it. We all are. Sharing my story has been central to my healing. It turned a private secret into part of a hero’s journey. It reduced my shame. It became a source of resilience. And underneath all of it, one truth kept getting clearer. There is nothing wrong with me. Something happened to me. Here is what I believe. Whatever our circumstances were, they are workable. And they are better in the light. Everybody has one of these stories. Talk to anyone long enough and you will find it. The spotlight in that movie was narrow. The work of bringing our stories into the open so we can heal together is wide. That is why I appreciate what Shawn Lesser and THE REAL Mental Health Foundation are doing. Giving men who were told to suffer in silence a place to talk and a bond to build. What if every AI startup in healthcare had to survive a 30% pay cut tomorrow? That question came up in our recent Forum Health Tech Industry Council meeting, and it stuck with me. Health system leaders discussed how reimbursement compression isn’t a “worst-case scenario,” it’s the baseline. As CMS and commercial payers push further into value-based care and tighten digital coverage codes, payment for each unit of care keeps shrinking. And ironically, AI is speeding it up. The more efficient documentation, diagnostics, and billing become, the less payers want to pay for them. If a startup’s business model depends on today’s reimbursement rates, it’s already on shaky ground. As Jay Rughani, Jane Rhee, and Julie Yoo point out in this a16z Bio + Health piece on “Infinite Healthcare,” healthcare today looks a lot like telecom decades ago—with a cost structure that reinforces a scarcity mindset. They argue that AI can shift healthcare from a system of scarcity to one of abundance, and I could not agree more. Many of us remember when phone calls were billed by the minute—more usage meant more cost. But as networks scaled, pricing shifted to unlimited plans. Usage surged and value expanded for both customers and providers. Our industry can follow the same path, but only if we shift to a payment model that aligns with the future we are trying to create. For decades, our healthcare system has been built on limits—limited clinicians, appointments, and coordination. Those limitations have normalized waiting and suffering—and added complexity. AI has the power to radically expand our capacity to care for people, helping us shift from simply reacting to illness to proactively supporting health every day. Technology alone will not change the system, though. Abundance cannot thrive inside a payment model that rewards episodic intervention instead of ongoing health management. If we want proactive, continuous care at scale, incentives must align with preventing illness, not just treating it. Scarcity shaped my dad and our family’s experience at the end of his life.
