Why it matters: Most of the organizations have not previously served Medicare beneficiaries and will bring additional technology-supported care options to help people manage chronic conditions like high blood pressure, diabetes, chronic pain and depression. What to expect: CMS is extending the initial application deadline to May 15, 2026, so that more organizations can participate in ACCESS when it launches on July 5, 2026; Medicare enrollment is required for participation but not to apply. The big picture: ACCESS will empower people with Medicare and their clinicians by offering services that are convenient, affordable and integrated with their existing care teams. They're helping aging parents navigate theirs. This The Guardian essay from Courtney Martin puts numbers behind a reality many families are already experiencing. Only 3–4% of Americans over 50 have long-term care insurance. At the same time, the author shares that her father's memory care costs $8,500 per month. What stood out to me is the article's focus on the "forgotten middle"—families who make too much to qualify for assistance (e.g., Medicaid), but not enough to comfortably afford the care they need. At Rosarium Health, we see this every day. Most families want to help a loved one remain safely at home for as long as possible. The challenge is that many don't begin planning for the home until after a fall, diagnosis, or hospitalization. It is encouraging to see conversations like this becoming more mainstream. Starting this summer, clinicians can refer their Medicare patients with chronic conditions to affordable, technology-enabled care options, while staying meaningfully involved in their care and billing for co-management. This means better coordination and better outcomes for patients. CMS has a dedicated ACCESS page with everything PCPs and referring clinicians need to know.Bill Evans vond dit interessantCalling Primary Care Providers, Health Centers, Pharmacists, and other clinicians: CMS launched a new webpage to explain how the ACCESS Model can help you support your patients with chronic conditions including diabetes, high blood pressure, chronic musculoskeletal pain, and depression: go.cms.gov/ACCESS. Starting on July 5, eligible patients can get technology-supported, disease management services such as lifestyle support, remote monitoring, wearables, coaching and medication management at low or no additional cost. The new ACCESS webpage also explains how the model will benefit providers, supporting seamless care coordination with model participants designed to complement your clinical care.

Thursday we announced that Omada Health will be an independent administrator in Eli Lilly and Company's Employer Connect program, adding another way for employers to pair GLP-1 access with our GLP-1 Care Track for coordinated, virtual lifestyle support before, during, and after medication. Employers have options now. Between PBM solutions, Lilly’s Employer Connect, and Omada’s own GLP‑1 Flex Care, there’s a full menu of GLP‑1 support pathways to fit the needs of their population and budget—while pairing medication with behavior change, muscle preservation, and cardiometabolic risk reduction through Omada’s program. While we’ve been building this menu out, we’ve also been listening to patients. What we heard: for many, access is no longer the bottleneck. Support is. In a survey of over 3,000 GLP-1 patients from Omada, we found patients still face stigma, support gaps, and rising out‑of‑pocket costs that threaten continuity of care.
