Most employers only rethink their benefits strategy at renewal. That’s the problem. For companies with 50–1,000 employees, healthcare is often a top 3 expense; yet the funding arrangement is treated like a once-and-done decision. Headcount changes, claims patterns shift, cash flow priorities evolve, but the funding model stays the same. When evaluation only happens at renewal, options are limited and decisions get rushed. Most employers are still evaluating unit cost, network discounts, and trend assumptions. While these are undoubtedly important, a very different problem is quietly driving plan performance. A small number of extremely large medical claims now account for a disproportionate share of total spend. These claims are getting bigger, more complex, and more common.
