When "I take my drugs daily" isn't the full story. In HIV care, a high viral load despite reported adherence is a clinical puzzle we encounter often. The missing piece is rarely willpower. It's life — inconsistent timing, alcohol use, travel, stigma, undisclosed status. That's where Enhanced Adherence Counseling (EAC) comes in. EAC is a structured, non-judgmental counseling process that helps patients and providers identify real barriers to adherence — and solve them together. ✅ Same-time daily dosing ✅ Managing side effects ✅ Reducing stigma-driven concealment ✅ Building support systems Then the outcome..Patients go from detectable to undetectable — protecting their health and stopping transmission. I've seen EAC change outcomes that medication alone couldn't fix. SAHCS has released new clinical guidance for the care of older persons living with HIV (OPWH). Today, 15% of adults living with HIV in the African region are aged 50 plus. By 2040, that figure is projected to reach 27% around 9.1 million people. ART has given people with HIV longer lives. The next challenge is ensuring those years are healthy ones, years lived with functional independence, cognitive health, mobility, and quality of life intact. I worked for LabCorp for several years. I had one patient who tested positive for HIV. It was a false positive though (which is uncommon).
