  • How much do expert radiologists agree when staging pancreatic cancer? Less than you might think, and more than a model should ignore. In our CURVAS-PDACVI challenge (presented at MICCAI 2025), we asked 5 independent radiologists to annotate 114 Pancreatic Ductal Adenocarcinoma (PDAC) CT scans: tumour boundary + 5 critical vascular structures. When we measured how much their annotations overlapped with each other, one thing became clear: → On average, experts agreed on only ~58% of the tumour area → The spread is enormous, some cases see near-perfect agreement, others near-zero → When we specifically looked at the most ambiguous cases - where experts barely agreed among themselves - we found that AI models trained to always give a confident answer simply have no mechanism to say "I'm not sure" when they should Looking at the segmentations in the picture, we can see it clearly: the same tumour, drawn by 5 experts, looks dramatically different at the vessel interface, precisely where the staging decision is made. Our takeaway: this variability isn't a problem to eliminate. It's a signal to learn from. That's the premise of the whole benchmark: preserve expert disagreement, don't erase it. And evaluate AI not just on how accurately it segments, but on whether it is appropriately uncertain in the same places human experts are. Clinical practice needs trustworthy AI - accurate, but also honest about its own uncertainty. That's what we are building at Sycai Medical.   • A new key milestone: LIRVEA™, Sycai Medical’s AI solution for the detection and longitudinal monitoring of liver lesions, has now been awarded the CE mark under the MDR as a Class IIb medical device, authorised for clinical use throughout Europe. Behind the CE mark lie the data: clinically proven sensitivity of 90.7% and specificity of 86.8%. In liver cancer, early diagnosis is one of the few real ways to improve prognosis, and results like these are what bring AI closer to the daily work of professionals treating abdominal cancer. Sycai Medical had the privilege of joining the global pancreatic cancer community at the European Pancreatic Club 2026 Meeting, sharing our vision on how Artificial Intelligence can play a meaningful role across the patient journey, from opportunistic detection to longitudinal follow-up and surgical planning. We did so alongside our partners at Medical Horizons S.r.l., with whom we shared these days of science, collaboration and innovation. A huge thank you to everyone who stopped by our booth and joined our conversations. It was a special honour to connect with some of the leading voices in pancreatology, and to feel the energy of a community so deeply committed to changing the reality of pancreatic cancer.   • Artificial intelligence will only have a real impact on pancreatic cancer if it is built on data, clinical evidence and international collaboration. That was the message from the Satellite Symposium organised today by Sycai Medical at the European Pancreatic Club Meeting (EPC 2026). Thank you to our keynote speaker, Dr Giovanni Marchegiani, MD, PhD, for setting the tone with clarity, depth and the kind of clinical insight that only comes from being at the forefront of pancreatic surgery.   • We are still at our stand at #EPC2026, the European Pancreatic Club conference in Istanbul, sharing case studies, clinical discussions and evidence on how artificial intelligence can support the detection of pancreatic lesions in real-world practice. Today, Dr Giacomo E.M. Rizzo, from La Maddalena S.p.A. - Dipartimento Oncologico di III Livello, explained how Sycai Medical helps radiologists at this cancer centre to detect pancreatic lesions. He reviewed several cases in which our tool has helped to identify lesions that had not been detected initially or to confirm the radiologist’s diagnosis. That is precisely the value we aim to provide: a second opinion integrated into the clinical workflow, capable of helping to detect lesions earlier, confirm findings and better support patient follow-up. Tomorrow, Dr Rizzo will present the poster: “A preliminary analysis of the diagnostic performance of a radiological artificial intelligence (AI) tool for the incidental detection of pancreatic lesions on computer tomography”.
