And eventually in building a company. Last weekend, back on campus for our Harvard Business School reunion, I found myself thinking about everything that's happened since we first met. Some of it looked the way we imagined. A lot of it didn't. Career changes, burnout, health issues, startups (others, now our own), years of fertility treatment, and then becoming parents, finally. So ironically, now that we're officially co-founders of a company, I realize it isn't that weird at all. When I chose a life partner, I was really choosing a co-founder then, too. They're the person sitting next to you when things are uncertain. When you're making decisions without enough information. When a setback turns into six months or three years. When you're trying to build something that feels impossible. And infertility was certainly that for us. At the time, I didn't realize how much those experiences would shape the work we'd eventually do. Looking back now, it's hard to separate the personal journey from the company we built. Heading back to campus brought back a lot of gratitude, perspective, and appreciation for the people we met fifteen years ago. But especially for the person who's been willing to keep building through every version of it.

The bet is simple: we've made AI single player, but teams are multiplayer Right now AI is built for builders. You pretty much have to be technical, or willing to burn hours learning the tools, to get anything real out of it. Everyone else gets left on the sidelines. Computers were like this once. Nerds built them in garages, until the personal computer put one on every desk and everyone got to play. And here's the catch: the people on the sidelines (the operators, the experts, the ones who actually run the business) are the ones who know how the business really works. AI is still in its garage era. Unfairly fixes that. It's a shared brain for your team: it gathers what everyone knows and lets anyone put AI to work on it. Everyone works off the same company AI, and the learnings compound over time. No terminal required. I'm walking in with the three hardest lessons from Invincible: → Team beats solo. I'm building with my good friend and genius Kilty McGowan (who hates when I say this 😃). → Build on momentum. There's never been more of a push for AI...and yet nobody has it figured out. → Make something people will pay for. Figuring out AI in companies is existential - those that do will survive, those that don't will get disrupted. It's a golden age for startups, and I'll be building this all in public. Thank you to Ashley Mastronardi for chatting about this with me on NYSE Live, and to Johanna Grossman, PhD for inviting me back! For years, building a virtual care business meant assembling your own clinical network, navigating licensing and compliance across 50 states, and standing up billing infrastructure from scratch. That barrier kept most brands out of healthcare entirely, even when their audiences were asking for it. Wellness brands, fitness platforms, creators, and clinics can now have a branded patient storefront, intake flows, and program configuration live in as little as 24 hours, built on OpenLoop's existing 50-state clinically compliant infrastructure. Full back-end operations, including provider credentialing and intake processing, are live within days. Here's why this matters: 1. MSOs are going to see historic profits over the next 2–3 years thanks to autonomous AI. And that's good for everyone. 2025 was a banner year for global healthcare PE acquisitions, with deals hitting a record $190B across 445 buyouts. MSOs were a big part of that activity. So what’s everyone seeing? Margin expansion at MSOs eventually hits a ceiling, and profit growth is critical to MSO success. Let’s start with why those ceilings exist in the first place. Private equity has already pulled most of the obvious levers in healthcare rollups: ☑ Shared services ☑ Vendor consolidation ☑ Offshoring ☑ Standardized SOPs Those moves worked… for a while. But over the past five years, many MSOs have been running into the same problem: margin expansion slows as scale increases. Labor constraints and rising costs start to dominate. Across MSOs, labor still represents ~55–60% of total operating expense. And roughly 30–40% of staff time and spend goes to non-clinical administrative work. As platforms add sites, that labor doesn’t stay flat. It often compounds, despite shared services and legacy tech, largely due to: - Workflow heterogeneity - Exceptions and edge cases fanning out at scale Without a new technology or innovation lever, the traditional playbooks were simply maxed out. Enter AI, specifically emerging autonomous AI platforms, which fundamentally change the math, especially at scale. I’ll share next why this unlocks the next tranche of profits, and why 2026–2028 will be breakout years for MSO margins. Hold on to your seats, ladies and gentlemen.

KLAS spoke with 35 Epic customers on what they think about Epic’s foray into AI scribes. Here were 5 interesting takeaways (courtesy of Becker’s) and my thoughts on them: 1️⃣ Some health systems expect Epic to deliver the same value as third party AI scribes, with better integrations and at a lower cost → This seems like an unreasonable expectation to me. The third parties will provide way more white glove change management support, support a broader diversity of specialty-focused customizations and manage edge cases way better. → I do agree with better integrations and a lower price point though - but you also get what you pay for - e.g. no white glove change management service, lower professional services costs. 2️⃣ Some health system executives are frustrated - they felt Epic endorsed some of the third party AI scribes and were caught by surprise that Epic developed scribing anyways → While some folks might point to how Epic often supports other third party products and then competes with them over time and perhaps this was foreseeable - I have empathy for health systems because this time was technically kind of different. → Epic Workshop and co-development with companies like Abridge - including supposedly getting warrants or shares in the company (before supposedly selling them earlier this year) - was very, very new. → I would guess that these Workshop partners were promoted more significantly than previous product partners, and now that the messaging might be changing, health systems were caught off guard. 3️⃣ 54% of health systems were Very Likely or Likely to choose Epic’s AI scribe within 2 years - with better integrations as the primary reason → The vast majority (85%) of the respondents to the KLAS survey do have some AI scribe already. So roughly speaking, ~50% of health systems who already have an AI Scribe are telling themselves they’ll switch to Epic’s AI scribe within 2 years. → I would be surprised if that many successfully, fully switch to Epic’s AI scribe within 2 years (especially since the scribe won’t come out for another 6-12 months), but we’ll see. 4️⃣ Health systems already on Microsoft (I assume Nuance DAX) were more likely to believe they’ll just move to Epic’s AI scribe → Not surprising at all - in their minds, the future Epic AI Scribe is basically just the current Microsoft/Nuance product. 5️⃣ While most health systems expect Epic will be their exclusive AI scribe long-term, some expect to need specialty-specific tools → It may be true that purpose-built AI Scribes for specialties with a lot more complexity and edge cases (e.g. oncology) will still be something Epic won’t touch. Personally I don’t think these answers are all that meaningful. Everything hinges on: → The quality of AI scribe that Epic releases and whether self-serve works → How much of a lead in product development the third party AI scribes build between now and then What do you think?

As Jay Rughani, Jane Rhee, and Julie Yoo point out in this a16z Bio + Health piece on “Infinite Healthcare,” healthcare today looks a lot like telecom decades ago—with a cost structure that reinforces a scarcity mindset. They argue that AI can shift healthcare from a system of scarcity to one of abundance, and I could not agree more. Many of us remember when phone calls were billed by the minute—more usage meant more cost. But as networks scaled, pricing shifted to unlimited plans. Usage surged and value expanded for both customers and providers. Our industry can follow the same path, but only if we shift to a payment model that aligns with the future we are trying to create. For decades, our healthcare system has been built on limits—limited clinicians, appointments, and coordination. Those limitations have normalized waiting and suffering—and added complexity. AI has the power to radically expand our capacity to care for people, helping us shift from simply reacting to illness to proactively supporting health every day. Technology alone will not change the system, though. Abundance cannot thrive inside a payment model that rewards episodic intervention instead of ongoing health management. If we want proactive, continuous care at scale, incentives must align with preventing illness, not just treating it. Scarcity shaped my dad and our family’s experience at the end of his life.
