Wilson, MD, PhD, CPE, FACEP reposted thisWithout health insurance, people often don’t receive the medical treatment they need until their illness becomes severe — and they’re forced to go to the emergency department. Untreated diabetes leads to kidney failure and blindness. High blood pressure causes a stroke, aortic dissection and heart failure. Nagging symptoms aren’t evaluated until the cancer that caused them has already spread. But here in Hillsborough County, we are fortunate that 35 years ago, a bipartisan coalition of political leaders, business groups, health care providers and educators worked together to design the Hillsborough County Health Care Plan. Today, more than 22,000 residents from every corner of #HillsboroughCounty, from Plant City in the east to Ruskin in the south and Carrollwood in the north, are enrolled in the plan, which is funded by a countywide half-cent sales tax. While the plan helps these residents directly, it also helps reduce health care costs for the entire county, funds local jobs and brings in millions in federal matching funds. The tax also funds money for emergency medical services, care for county prisoners and provides the revenue for other mandated county costs. Last year, an independent economic analysis showed that the plan generates $1.53 in community benefit for every $1 of tax funding, a total of more than $291 million in benefits. It also supports at least 500 jobs in health care. Those are not abstract figures; they reflect real dollars, real jobs and a stronger local economy. There are well-intended suggestions that the plan should be scrapped. But the Hillsborough County Health Care Plan is worth protecting. It is a practical investment in prevention, workforce stability and community prosperity. It is a smart, locally designed solution for people who are uninsured, need a bridge to health care and do not have private insurance, Medicare or Medicaid. The board provides recommendations on the Hillsborough County Healthcare Plan (safety net coverage for our most vulnerable). I joined the board in 2011 and am one of the longest-serving members. I appreciate the confidence my board colleagues have placed in my leadership and am deeply humbled to step into the Chair role as founding board member John Curran transitions out. I'm proud that my first official act as Chair was to name Dr. Curran emeritus Chair by unanimous vote. Fittingly, thanks to the vision of Commissioner Gwen Myers, the board will also now be named in his honor. Wilson, MD, PhD, CPE, FACEP shared thisThere is a role for AI augmentation of the acute clinical encounter, and this manuscript from Science does demonstrate that potential tools are improving. However, the media headlines (AI beats doctors) are of course splashier than reality — this was not a prospective ED-based study measuring EM physician performance, and in any case, diagnosis is rarely the crux of the problem we are trying to solve during the acute encounter. This is a dataset evaluation, with both AI and physicians generating differentials offline at predefined touchpoints. No head-to-head, real-time, in-the-moment EM workflow was tested. While the study makes claims about EM, it is critical to note that the comparator physicians were internal medicine attendings — not emergency medicine. If the implication is that this informs ED triage and how precision evolves as a case unfolds, the comparator specialty matters. EM clinicians reason differently because the task is different. The ED encounter is largely about ruling out dangerous disease (ACS, PE, SAH, sepsis, stroke), shaped by social and system factors as much as pathophysiology. While House made it seem like "the diagnosis" is the climax of medicine, in real EM practice the rule-out — and then care coordination — is usually the work. At the same time, the discipline carries a public health safety-net function: for many patients, unscheduled ED care is where they actually access — and demand — healthcare delivery. If we are patient-facing, we accept that reality and build around it. What is the point of spending money and time on alternatives when the 24/7 hub already works well for many of these patients? Reducing the expectation mismatch around what actually walks through ED doors — complicated unscheduled care that needs coordination, not just sepsis, strokes, and STEMIs — may also reduce physician burnout. And embracing that broader role creates a real opening for sharper, and even opportunistic, diagnostics at the point of contact. Physician-in-the-loop LLMs will likely help here. As ED disposition becomes more granular — admit vs. transition clinic vs. hospital-at-home vs. tele-coordination — earlier, more precise diagnostic support could meaningfully change patient experience and care pathways. The issue of the false positive in HIV testing was one that we uncovered secondary to our high volume and acute ED based non-targeted testing program.Jason W. Wilson, MD, PhD, CPE, FACEP shared this🤯 1 in 6 reactive screens came back positive for HIV, when confirmatory testing found they weren't in our ED based screening program. Why? That question drove our team's new publication in AIDS Research and Therapy, out this month. Fourth-generation HIV antigen/antibody assays are exceptional tools: >99% sensitivity, >99% specificity, and the backbone of ED-based screening programs like ours. But exceptional isn't perfect. Over five years of universal screening in our emergency department, we found that 1 in 6 reactive screens were ultimately false positives. 116 patients whose confirmatory testing told a different story. In a follow-up case series of 13 patients, we looked at what was happening clinically when the false positive occurred. The pattern that emerged: immune activation. Acute infections (including COVID-19, hepatitis A, and bacterial sepsis), autoimmune disease, hematologic conditions, and liver disease all showed up repeatedly. Most false reactivity resolved on repeat testing once the acute process cleared, but persistent false positivity was more common in chronic conditions, especially liver disease. This matters because the ED is increasingly where HIV screening happens, especially for patients who aren't regularly connected to primary care. Knowing what can trip up the algorithm helps clinicians counsel patients more accurately in that vulnerable window between screen and confirmation. She built most our local EMS systems (aeromed, TFR, EMS fellowship) and trained hundreds of medical students and many residents. She was a strong female personality in a male dominated world of medicine, EM, EMS. She was even instrumental in my desire to go to medical school when I was in grad school, watching ER and thinking about a change (she is in “Life in the ER”, 2001) - she exhibited the tough but cool traits I came to know as quintessential for EM. I even got to train with her when she still saw adults, before she decided they were too smelly (ha) and just wanted to take care of the kiddos. Thanks Carrubba (she preferred that over “Dr.”). Wilson, MD, PhD, CPE, FACEP liked this10 years ago today, I joined the faculty at the Baptist Health Herbert Wertheim Cancer Institute (the former Miami Cancer Institute). At the time, the cancer center building was still under construction, and the Department of Radiation Oncology as it exists today had not yet been established. When Minesh Mehta recruited me, he told me that we had the opportunity to build something special. Looking back a decade later, he was absolutely right. I am incredibly proud of what we have built from the ground up, and to work at a world class institution defined by a commitment to quality, innovation, outstanding patient care, and exceptional clinical outcomes. We have become an internationally recognized leader in advanced radiation therapy, and have been fortunate to recruit faculty who not only lead innovative clinical trials but often design them, helping shape the future of cancer care. I am grateful to Minesh Mehta, the late Michael Zinner, Leonard Kalman, and so many others throughout the Baptist Health system for their trust, mentorship, and support. Most importantly, I would not be where I am today without the team around me. The greatest part of this journey has been working alongside colleagues who inspire me, challenge me, support me, and make our shared success possible. I am deeply grateful to work side by side with our remarkable staff as we care for patients who come to us from across Florida and far beyond. Wilson, MD, PhD, CPE, FACEP liked thisMy beautiful wife nailed this cross to the tree where a man was struck by lightning in front of our home on Monday. She got to him within a minute of being struck and he was not breathing and had no pulse. 25 minutes of her doing nonstop CPR, his pulse came and went throughout the duration of her administering aid, heavy rain and lightning surrounding her, and as I rushed to get home and pulled up to her pumping on the man’s chest I asked her what she needed. She looked at me, soaked in rain, blisters on her hands and knees, makeup smeared down her face, lightning cracking all around and calmly said “prayer”. By the grace of God the man survived and my wife has taken ZERO credit for doing something most would never be able to do. She simply says, “I was just a vessel for God’s work….nothing more.” She put this cross on the tree to remind us all of how important it is to use our free will to choose obedience to God’s calling even when it puts us in harm’s way and we don’t want to go. HIV and HCV Testing and LTC in the ED
Background
In 2006 the CDC changed their HIV screening recommendations from testing only high-risk targeted groups to recommending one lifetime HIV test for all patients presenting at emergency departments. In August 2014, Florida statute 381.004 allowed opt out consenting for HIV testing, and this paved the way for our current emergency department HIV and hepatitis C screening program at TGH. Most recently, Florida house bill 1041 further supported opt-out testing in emergency departments.
Testing details
We currently test between 21% and 25% of total ED patients per day. Positive screening patients receive confirmatory testing (for hepatitis C, a PCR viral load test; for HIV, the Geenius antibody test and PCR viral load test). Patients without insurance covering these testing costs are subsidized by a research grant (FOCUS Project) we received in April 2016 from Gilead Sciences, Inc.
Our linkage to care team works with community partners to help newly diagnosed and previous positives who had been lost to care. We help patients apply for funding, make clinic appointments, and transition to care. -

Program involved 20-30 hour per week internship at Hernando County Health Department. During that time, our team (myself, a public health student and a nursing student) developed and updated a STD prevention program. This work culminated in a capstone community event (a youth dance at a local recreation center) and an academic presentation to public health faculty and AHEC staff.
