NEW DATA RELEASED TODAY!! 📣 I am so proud of the NC Department of Health and Human Services and NC Medicaid for their leadership! This effort definitively shows that using Medicaid funding to pay for targeted, evidence-based services such as housing, food, transportation, and interpersonal safety, improves health outcomes and reduces Medicaid healthcare costs. The evidence is compelling: 💰 Medicaid spending decreased by an average of $164 per person each month. This savings is inclusive of the cost of service and administrative overhead. 🩺 HOP participants had significantly fewer emergency department visits and hospital stays, and more visits with outpatient providers compared to what would have happened without HOP. 👨👩👧👦 89% of participants surveyed at 6 months said HOP improved their health and a positive impact on their or their children’s health. 🤳 Strong communication across healthcare providers, network leads who served as regional connectors, community organizations, and participants helped improve outreach, engagement, and enrollment. HOP was run in the rural parts of our state (not in our cities) --> so every state working to deploy their rural health transformation funding should be looking at this effort as part of their strategy. Improve health and reduce Medicaid costs! The federal government and states should be running to put this in place everywhere. There are so few solutions where you can give people more services, improve their health AND save money (inclusive of cost of the services and administrative costs!!!) Taylor Justice reposted thisTaylor Justice reposted thistoday we're releasing Aavia's first data report! 250M+ lived-experience data points. years in the making. 📊 ~ if you are building, funding, or scaling anything that touches women's health and you want to understand what the data actually shows, this report is for you. ~ i could lead with the clinical findings. and they're significant. but the thing that stopped me was this: the most common word in Aavia's community forum is ~HELP~ 1 in every 4 posts! not "track." not "optimize." help. these are not passive members logging data. these are women in the middle of a health experience they cannot name, reaching for language before the healthcare system has given them any. and here's what the healthcare system is doing while women wait: - taking an average of 12 years to diagnose PMDD. Aavia flags it in under 100 days. - giving women fewer than 10 healthcare touchpoints per year. Taylor Justice posted thisI am proud to see Unite Us recognized by Centers for Medicare & Medicaid Services as an approved vendor on the federal GSA Schedule. We have a shared goal: to lay the foundation for a more modern, integrated Medicaid system. This matters because speed matters. Right now, states are challenged to meet the needs of multiple initiatives. Making it easier to access approved, trusted technology helps states move faster and get the right digital tools in place when they’re needed most. Unite Us has a long history of partnering with state and local governments to strengthen health and human services infrastructure, and this is part of that commitment. Looking ahead, we’re excited to continue supporting states as they leverage digital solutions to build stronger, more connected systems of care that scale.

Taylor Justice liked thisTaylor Justice liked thisCMS has released its proposed 2027 Medicare Physician Fee Schedule, and it is worth paying attention to. While this 716-page proposed rule focuses heavily on payment policy, its direction is clear. CMS is continuing to accelerate the transition to value-based care. CMS is increasingly rewarding providers who improve outcomes, coordinate care, keep patients healthier, and can demonstrate measurable results. Among the most significant proposals that I will highlight are these 9: 1) Expanded incentives for prevention, care coordination, and chronic disease management by allowing primary care providers to receive prospective, population-based payments. 2) Greater flexibility for Accountable Care Organizations (ACOs) to reduce or eliminate patient cost-sharing for selected services that are expected to improve outcomes and lower overall Medicare spending. 3) Better integration of behavioral health into primary care by allowing ACOs to use shared medical appointments. 4) Sunsetting the traditional MIPS program (after the 2028 performance year) and replacing it with a streamlined Value Pathway program that simplifies quality reporting and emphasizes value-based performance measures. 5) Expanding the use of digital quality measures generated directly from electronic health records, reducing reliance on manual reporting. 6) Making telehealth and virtual care a more permanent part of Medicare payment policy, rather than treating them as temporary pandemic-era exceptions. 7) Shifting physician reimbursement toward primary care and chronic disease management. While overall physician fee schedule payments would decline as the current statutory payment adjustment expires, CMS proposes higher payments for providers delivering primary care and longitudinal chronic care management. 8) Signaling that digital health technologies and AI-enabled tools may increasingly become part of Medicare’s payment model, rather than costs that providers must absorb within existing reimbursement. 9) Redesigning reimbursement for remote physiologic monitoring through four new HCPCS G-codes, with payment generally limited to monitoring services performed by clinical staff employed by the billing practice. Recently, Unite Us’ own Kelsey Hague, Jordan Murray and Kylee Dick joined SSM Health and an incredible group of local organizations at SSM Health St. Agnes Hospital in Fond du Lac County to strengthen coordinated care efforts. At the center of this collaboration, the SSM Health Samaritan Clinic and Case Management teams identify patients' social needs and coordinate referrals through Unite Us. By working closely with dedicated local partners like ADVOCAP, CHARIS PREGNANCY HELP CENTER INC, St. Katherine Drexel Shelter, SOLUTIONS CENTER SHELTER AND SUPPORT SERVICES INC, Holy Family Warming Shelter, Fondy Food Pantry, and Fond du Lac County Health Department’s Save A Smile program, they help connect individuals to the right services so they can easily navigate the support available to them. Through demonstrations and collaborative discussions, participants explored how shared networks enhance communication between healthcare providers and community organizations. The focus remains entirely on helping individuals experiencing homelessness or food insecurity receive timely, integrated care beyond clinical walls. It is clear that too often veterans leave the service, isolated, without the resources, and without the community, they so desperately need and deserve. We hope to do our small part to change that through partnership and collective effort. By bringing together a diverse group of like-minded organizations we seek to create a welcoming, low-pressure place for veterans to learn about services and opportunities available to them, build friendships, and forge a connection to their local community. In conjunction with that, we take pride in honoring others that serve their communities in a variety of fashions, including first responders, volunteers, and nonprofits. The podcast ties these individual events into a larger, interconnected, virtual community. By telling encouraging and thought-provoking stories of service, we can encourage all veterans to find new creative outlets, fulfilling ways to give back, and a sense of belonging.
