From my vantage point, several themes have crystallized this year: Strategic Intent Matters Most Innovation must be anchored in clear outcomes. Leadership as a Force Multiplier True transformation happens when leaders empower teams to adapt, learn, and innovate together, fostering an environment where people feel supported to lead and grow. Responsible, Scalable AI Adoption In healthcare, where trust is essential, governance, transparency, and ethical use of AI are foundational to meaningful adoption and impact. Healthcare’s Data Advantage Digital platforms and advanced analytics are enabling us to accelerate insights and improve care decisions, from predictive population health to integrated, data-led care pathways. Importantly, at @QuestDiagnostics, we are actively embracing these themes, helping transform capabilities, strengthen outcomes, and support the broader mission of advancing care. We're having all the wrong conversations on the future of EHRs (SORs) with AI right now. EHR/EMR companies, their investors, and the talent who all financially rely on the systems existing as it does today are all telling you the same talking points. 1. No one will vibe code their own EHR/EMR. It's too complicated and not part of their business model. So why will they build it? 2. Their data as the system of record is far too valuable to not be used by customers. 3. They own the distribution therefore they can't be replaced. The truth: They're half right. But here's where they're wrong. User behavior is not the same. EMR/EHRs are betting that EVERYTHING about the end user stays the same. With SaaS due to consolidation the end user wasn't the doctor or patient, it was administrators, developers, product managers, digital health tech CEOs. AI changes all of this. The true end user (doctors and patients) are now using AI without permission and their employment status does not matter. Meaning that this new user behavior WILL influence AI usage and product adoption, not board rooms or CIO/CMIOs of the past. Now of course, healthcare systems or other organizations can attempt to ban AI but we all know how that will actually play out in practice. (Hence the term ShadowAI). So going back to will EHRs/EMRs get replaced in an AI first era of medicine or healthcare? The answer is 100%. But it's not because they vibe coded their own software, it's because they now have the agency to decide what AI tools they use for treat and manage patient care. One of the most interesting debates I heard today at HIMSS... “Is it time to trust LLMs over interoperability standards?” For decades, healthcare IT has tried to solve interoperability the same way: Build structured standards → translate data → move it between systems of record. FHIR. HL7. Interfaces. Mapping tables. Endless implementation cycles. But the panel posed a provocative idea: What if LLMs flip that model entirely? Instead of forcing every piece of healthcare data into rigid structure… 👉 Let LLMs work directly with the messy, unstructured reality of healthcare. #RaihanReacts to some themes that stood out: 1. LLMs make massive EHR outputs digestible One panelist joked that they “spent yesterday afternoon in Claude Code building their own apps.” That comment actually reflects a real shift happening across healthcare IT. Clinicians and operators are suddenly able to take: • thousands of lines of chart data • referral notes • discharge summaries • scanned PDFs • messy clinical documentation …and have LLMs summarize, extract, and explain it in seconds. The result: 👉 EHR data becomes understandable and actionable, not just stored. 2. Unstructured data may actually be healthcare’s natural format Healthcare has always struggled to structure everything. But maybe that was the wrong goal. Clinical care is inherently messy: • free-text notes • patient narratives • imaging reports • external records • faxes (still…) LLMs are the first tech that actually thrives in this environment. Instead of forcing the system to change…the tech adapts to healthcare’s complexity. 3. Accuracy and governance are the real challenge Of course, trusting LLMs introduces a critical question: How do we verify the outputs? Several panelists talked about the need for: • monitoring frameworks • validation layers • auditability of summaries • “toggle switches” to turn LLM workflows on or off 👉 This is going to be one of the major operational challenges of AI adoption in healthcare. Not whether models can summarize data. But whether orgs can safely trust those summaries at scale. 4. Early operational wins are already emerging A few examples discussed: • Faster prescription workflows • Referral turnaround improvements • Automated chart summarization for intake and review These are small changes individually. But across a health system, they add up to thousands of hours of operational efficiency. My take: - Interoperability standards are not going away. - Systems of record will still matter. - But LLMs may become the universal interface layer that sits on top of them. From Data Exchange to intelligent Orchestration: The future of #HealthcareInteroperability in the age of #AI Healthcare interoperability is moving from a problem of data exchange to one of data understanding and orchestration. Traditionally, standards like HL7 and FHIR have focused on structuring and transporting information between systems. They solved syntax and, to some extent, semantics but not intent. Data could move, but it still required significant human or rules-based effort to interpret, map, and act upon. The emergence of AI, particularly LLMs and GenAI, begins to shift this paradigm. These models can ingest heterogeneous healthcare data for E.g. structured (FHIR resources), semi-structured (HL7 messages), and unstructured (clinical notes) and normalize meaning dynamically. Instead of rigid interface mappings and ETL pipelines, we start to see adaptive interpretation layers that understand context, infer relationships, and even reconcile discrepancies across systems in near real time. This changes interoperability in three fundamental ways: From Static Mapping to Dynamic Understanding: LLM's reduce dependency on pre-defined mappings by learning representations of clinical concepts across formats. This significantly accelerates onboarding of new systems and reduces interface maintenance overhead. From Data Exchange to Workflow Orchestration: Interoperability will no longer stop at “data delivered.” AI agents can trigger downstream actions such as clinical workflows, prior authorizations, care coordination based on interpreted intent, not just received data. From System-Centric to Patient-Centric Context: AI can synthesize longitudinal patient data across fragmented systems into coherent narratives, enabling more meaningful interoperability that aligns with clinical decision making rather than system boundaries. However, this evolution does not replace standards like FHIR and it builds on them. Structured standards remain essential as a foundation of trust, governance, and consistency. AI acts as a cognitive layer on top, filling the gaps where rigid schemas fall short. In the near term, interoperability will be hybrid: standards for reliability, AI for intelligence. Over time, the balance will shift toward systems that are less tightly coupled by predefined interfaces and more loosely coordinated through shared understanding. The discussion highlights how AI agents are evolving from simple tools to digital team members—streamlining HR, supply chain, and R&D processes while unlocking cost savings and productivity gains. Early adopters like Medtronic and Siemens are already demonstrating measurable impact through AI-enabled workflows and digital asset management. This reflects the broader trend—AI is moving beyond isolated use cases toward enterprise-wide operational transformation in companies large and not-so-large.
