Stephen Tweed shared thisHow is Corporate Culture affecting the future of Home Care? For the past several months, I've been talking with Founders and CEOs of companies in the top tier of the Home Care Industry. My interviews are around their process for strategic planning, facilitating leadership and board retreats, and crafting their company culture. While there are some very interesting insights coming from these conversations, one aspect of Corporate Culture that is quite interesting is the number of companies who are making a conscious effort to shift from a Customer First Culture to a Caregiver First Culture. What does this mean? It means that companies are putting the interests and preferences of their caregivers ahead of the desires of their clients and primary family caregivers. For most business leaders, that concept is heretical. The idea of putting employees before customers is contrary to all those who believe "The Customer Comes First", or "The Customer is always right." However, data over the past decade shows that the biggest pain point for Home Care CEOs is the caregiver shortage, and the barriers to business growth that comes from this shortage. They are talking about their biggest issues being caregiver recruiting and caregiver retention. However, when we look at where most CEOs spend their time and energy, it is on sales and marketing, and technology, and not caregiver recruiting and retention. However, there is a growing number of leading CEOs who are working with their leadership teams to craft a Caregiver First Culture. Their premise is that if you create a great place to work, and you take care of your caregivers, they will take care of your customers. They recognize that there are simply not enough caregivers in the industry to meet the needs of an aging population, and the only way to grow and prosper is to create a culture that attracts the very best caregivers to your company, and away from your competitors. What to do you think about this premise? Can companies really serve their clients better by attracting and retaining the best caregivers? How will senior leaders and managers respond to this shift in thinking?

Stephen Tweed shared thisI don't know about you, but I love summer school. For all five summers when I was in undergraduate and graduate school, I went to classes. So I'm delighted to be teaching again this summer ... Corporate Culture in the Master of Arts in Communication and Business Leadership at the Nido R. Qubein School of Communication at High Point University in High Point NC. This past week, I had the opportunity to conduct an interview for my students with Mark Baiada, the Founder and Chairman of the Board of Bayada Home Health Care, the largest, family founded, not-for-profit Home Health, Hospice, and Home Care company in the world. Last year Bayada generated $2.5 Billion in annul revenue, and served their Millionth client. Mark and I have known each other for many years, and have had a number of breakfast conversations about how he started the company fifty one years ago, and how he has spent those five decades crafting the culture of this amazing organization. One of the artifacts, or visible signs, of the corporate culture at Bayada is their "culture card" - The Bayada Way. This is a four-fold plastic card the size of a business card which shows the mission, vision, and values of the company. On the values side, it also lists the key actions or behaviors that demonstrate each of their three core values. Every employee is given a culture card, and most of them carry the card in their pocket every day as a symbol of what it means to live the values. When we talked about the three core values for the students, Mark reached into his pocket and pulled out his culture card and read the mission, vision, and values. I reached into my desk drawer and pulled my copy of the culture card. What about your organization and your company culture? What are the core values that guide actions and decisions, and that are the foundation of your culture? What are the behaviors that you expect in order to live the values? What are the visible signs of your organization's culture?

Stephen Tweed shared thisLast week, I posted an image of Dr. Edgar Schein's Model of Organizational Culture and talked about the work I am doing in Corporate Culture in the graduate school of communication at High Point University. That conversation generated a lot of interest and discussion about the practical application of corporate culture in Home Care organizations. That caused me to think about a complementary model that defines Corporate Culture as "The Way We Do Things Around Here," and suggests four factors that shape the culture of any organization: 1. The Leadership Style of the CEO 2. The Core Values that guide actions and decisions. 3. The Behavior we Expect 4. Not there to check anyone's work. There to learn one more thing before handing back the badge. Most executives spend their final weeks briefing a successor and clearing out an office. This one sat in on onboarding sessions meant for someone twenty years younger. Rank has a way of closing doors that curiosity used to keep open. Fewer people correct you the higher you climb. Correction fades. Growth follows it out the door. ⤵️ A few habits worth borrowing before the title gets in the way: ➣ Sit in on a session meant for someone junior to you, at least once a quarter. ➣ Ask a question you already know the answer to. Listen for what's different this time. ➣ A book, article, or course with nothing to do with your role, always in progress. ➣ Let a junior colleague teach you something in front of the team, and say thank you out loud. ➣ Track what surprised you this month. Nothing surprising is the warning sign. That new hire already tells this story when someone asks about their first month. The next new hire on your team is learning something from you right now. Stephen Tweed reacted on thisStephen Tweed reacted on thisWell, This Is Not the Invitation I Thought It Was Y’all, I got an invitation in the mail today. Not a bill. Not a coupon for gutter guards. An actual invitation. Thick paper. Fancy border. Elegant lettering. The kind of envelope that makes a woman sit up straighter and wonder who has finally recognized that she belongs somewhere with assigned seating and butter shaped like a flower. I opened it expecting a wedding, an awards dinner, perhaps a mysterious wealthy relative announcing that I had inherited a coastal estate. The front said, “You Are Invited,” and honey, I was already mentally choosing my outfit. I thought, “Well, somebody finally put some respect on my name.” Then I turned the page. It was an invitation to a free gourmet dinner exclusively for people with urinary incontinence and bladder leakage. I have never gone from “Who’s getting married?” to “Apparently, my pelvic floor has been discussing me behind my back” so quickly in my life. And they made it sound so exclusive. You don’t need tickets. You don’t need to bring a friend. You don’t need to know the hostess. You just need to sneeze unexpectedly in a Target parking lot, and apparently you’re on the list. Nothing prepares a woman for reaching the age when her social calendar starts being managed by her organs. One day you’re getting invited to bridal showers and cocktail parties. The next day your bladder has secured you dinner reservations at Sammy G’s Tavern. The invitation even said space was limited, which felt unnecessarily competitive. Am I supposed to call immediately? Is there a waiting list? Do I need documentation? Will there be a small audition involving a trampoline? And let’s discuss the menu. They’re inviting a room full of women with bladder leakage to a dinner where beverages will presumably be served. That is not hospitality. That is a live demonstration. Still, free gourmet dinner is free gourmet dinner. I may not have inherited an estate, but apparently I have inherited my mother’s pelvic floor, and evidently that comes with chicken. So yes, I will be attending. I’ll be the woman dressed like it’s an awards banquet, sitting closest to the restroom, refusing the second glass of tea, and telling everybody I was personally selected. Because I was. By my bladder.

Let’s get one thing straight — EISEP is not a new program. It’s a long-standing, county-administered program that funds non-medical in-home supports for older adults who don’t qualify for Medicaid. In the last few days, several licensed home care providers reached out asking, “What is EISEP?” What struck me wasn’t the question. It was the reminder of how easy it is—when you’re deep in day-to-day operations—to lose sight of adjacent programs and growth opportunities. A recent NYS DOH DAL simply clarified that physician orders are not required for these services. Compliance keeps agencies standing. Market awareness keeps them growing. And growth today requires lifting your head up just high enough to see what’s next.

Nonprofit organizations depend on engaged board members to help drive their missions forward. Here are a few of the benefits of giving your time to the nonprofit community through board service: Networking: Over the course of a 3–6 year term, you get to know fellow board members extremely well. As people rotate on and off, your professional network can grow significantly. Name and brand recognition: Your company name and the products or services you offer become known and trusted by fellow board members and their networks. Skill development: Regardless of your role or industry, board service stretches you beyond your core expertise: leading meetings, reviewing financials, strategic planning, HR issues, fundraising, event planning, marketing, legal documents, audits, and more. Behavioral healthcare has changed little since 2020, despite $6 billion in investments. Start-ups benefited, but did this historic funding bubble advance the field? Let’s assess four operational domains: VIDEOCONFERENCING. Video sessions are here to stay. While Skype was launched long ago, it was the pandemic that alleviated privacy concerns and spurred widespread adoption. This simple innovation improves care access. CURATED PRACTITIONER NETWORKS. We witnessed the dismal failure of managed networks (with MBHOs) decades ago. Nonetheless, new companies (e.g., Lyra, Spring) have soared in value with inflated network claims. Today’s difference is that proprietary, overblown technology feeds their network hype. DIGITAL THERAPEUTICS. Born in rudimentary form with the internet itself, online tools continue to improve every year. These services offer value, however limited. MEASUREMENT-BASED CARE (MBC). MBC vendors emerged in the 1990s, leveraging decades of therapy research. Investors have funded MBC vendors, but broad adoption requires health plans to adequately fund ongoing services. We might ask, was a superior investment neglected? Consider a complex, yet transformative, care integration strategy—placing therapists in every primary care setting to provide brief interventions. Sadly, investors prefer technology. Institutional changes are low-margin, though essential, innovations.
