    This paper outlines a small study undertaken to assess user perspectives on the concept of social prescription services. Social prescribing is a mechanism linking patients in primary care with non-medical sources of support within the community. The work presented here supports the idea of patients becoming ‘active partners’ by providing health literature that is designed to suit their health literacy along with a service which introduces patients to actual programmes and services in their local area which suit their specific condition. By using innovative digital technology patient engagement is encouraged leading to greater self-care and independence in relation to long-term condition management.     The findings in this report will be used to inform an action plan that will be developed in collaboration with the key stakeholders who can influence the age friendliness of this region.

    Over 200 older people had their say on what makes Derry a great place to grow older. They shared their thoughts, fears and aspirations about growing older here and outlined the challenges they face. They identified solutions to overcome the challenges and all agreed that yes, this is a good place to grow…

    The findings in this report will be used to inform an action plan that will be developed in collaboration with the key stakeholders who can influence the age friendliness of this region.

They identified solutions to overcome the challenges and all agreed that yes, this is a good place to grow older; we just need; ‘all the relevant sectors and organisations to commit to focusing on what can be improved’.
