Carine Carmy hat dies geteiltFor years, the build vs. buy decision in tech-enabled services was almost always: buy. Not because buying was better. Because building required an engineering team most companies couldn't justify. AI changed that math completely. Our CTO Nick Kavassalis explains: in the last few months, our two-person engineering team has shipped more than we built in the previous two years. Multiple tools built in weeks, not quarters. Hundreds of dollars in cost, not hundreds of thousands. We built a set of clinician and admin facing tools that improve the team experience and enable us to deliver even better outcomes. The unlock wasn't just speed. It was specificity. Not generic PT, but pelvic floor therapy. Built on the learnings from hundreds of thousands of patient interactions. No vendor can build to that. But we can. The question we keep asking: why would we license a generic tool when we can build something trained on our own clinical patterns, tuned to our own care protocols, and improved in real time by the clinicians using it every day? We're not building tech to replace our clinicians. Carine Carmy hat dies geteiltShow versus Tell. For a long time, I've been doing a lot of telling. Shouting from the rooftops about the power of pelvic floor therapy. But for many people, it's hard to understand what this actually looks like, especially when you add "virtual" to the mix. If you don't know what your pelvic floor is and you haven't done much PT, "virtual pelvic floor therapy" is a string of words that can make your head spin. So yesterday I got to SHOW and TELL. My amazing colleague Sarah Clampett and I joined Jacqueline Buckingham and her team MENO. Jacqueline didn't just hear about pelvic floor therapy, she experienced it firsthand. One thing you'll notice: no nudity. Because pelvic floor therapy isn't just internal. It's about connecting to all the muscles in your pelvis and everything around it - hips, glutes, core - to build real stability, strength, coordination, and range. Then she spent years fighting a second battle -- one her oncologists weren't trained for, her insurance wouldn't cover, and her body couldn't wait on. The space between survive and thrive is systemic failure. We celebrate survival. We put up the pink ribbons and ring the bell and call it a win. We don't talk about what chemo does to the pelvic floor. About the bowel dysfunction, the sexual pain, the muscles that have been through surgery and radiation and trauma and are now screaming... in ways that oncologists and gynecologists aren't trained to treat, and don't have the time to explore even if they were. Pleasure isn't a luxury. It's a measure of health. Her $10,812 went to: a pelvic pain specialist at $650/visit, pelvic floor PT at $245/session, endometriosis excision surgery, pelvic Botox at $4,611, compounding pharmacies, a vibrating dilator, oxytocin nasal spray. All out of pocket. All, incidentally, things that work. The system's alternative? 15 minute in-network PT sessions. Nothing improved. And here's what kills me: it doesn't have to be this way. At Origin, we fight for 45-minute visits covered by insurance. You shouldn't have to choose between care you can afford and care that actually works. Lily writes, "I'm not a full-time patient anymore, but I'm starting to accept that I'll be part-time forever." Part-time patient. As a finish line. After surviving cancer. This shook me and is a story I know far too well from other patients we support. (s/o to Marisa Peters for all your advocacy!) I've told my own story about painful sex publicly: a decade of misdiagnoses, of being told "that's just how it is," before pelvic floor PT changed my life in weeks. That experience built Origin. But Lily's story reminds me that for cancer survivors, the gap is even wider. You spend years fighting for your life. You win. And then you're on your own. Let's do better.

Carine Carmy hat dies geteilt"It's a dumpster fire of brokenness." Rachel Rubin's synopsis of the state of our industry is something I feel in my bones. For nearly a decade in my 20s, I had painful sex. Saw provider after provider. Was told to relax, see a therapist, to "have a glass of wine." Nobody mentioned pelvic floor PT until my now Co-founder Nona told me about her postpartum recovery. I was better within weeks. This Guardian piece by Carmen Rising on the cost of vulvovaginal pain brought it all back. 1 in 4 women affected. $31–72B in annual economic burden. Patients spending $12,000, $40,000 out of pocket... just to be heard and properly treated. Three things driving this crisis: 1️⃣ The training gap is enormous. Nearly 60% of patients saw three or more providers before getting a diagnosis. This isn't a rare disease. It's a systemic failure in how we train doctors to treat women's bodies. 2️⃣ Pelvic floor PT is still a luxury. Out-of-network, expensive, out of reach for anyone without significant means. That's a payer failure, not a personal one. Pain is whole-body. It wrecks mental health, relationships, work. Treating the muscle without treating the person is incomplete medicine, and pelvic floor therapy is just one of many specialties that lack adequate coverage here. 3️⃣ I co-founded Origin because I'd been that patient. Making this care accessible - in-network, affordable, not concierge - has been the mission since day one. Carine Carmy hat dies geteiltThe healthcare system treats what it can bill for. That's why we screen for postpartum depression -- and ignore everything else that breaks after birth. Sejal Tyle Hathi, MD, MBA wrote a devastating piece in the NYT this week. She's a physician. She runs her state's health agency. After delivery, she had a third-degree tear, urinary incontinence, and a diastasis recti so severe that lifting her daughter from the crib was a deliberate act. She was discharged at six weeks with no one owning her recovery, and had to assemble her own care from scratch. If she couldn't navigate it, the system isn't broken for some women. It's broken by design. Postpartum depression screening exists at scale because someone did the hard work of building the infrastructure. Specifically, a validated tool, clinical guidelines, a reimbursement code. The signal to doctors and patients became: this matters enough to ask about. Physical recovery never got that signal. No standard screen for pelvic floor dysfunction. No routine question about incontinence or pain with sex or prolapse. There's simply no code that makes an OB or PCP pause and ask. Awareness is rising. Doctors are beginning to ask the questions and normalize conversations around postpartum recovery. We're partnering with 10,000+ referring doctors at Origin. That's a start! But one provider doing it isn't a system. A CPT code is a system. So my question: - Who built the postpartum depression screening infrastructure? Who was in that room? - And is anyone working on the physical recovery equivalent?? Because of that hustle, I don't take for granted the joy of zooming out. A few weeks ago our leadership team spend a few days together talking big pic: Ways of working. Operating at scale. How we stay aligned as a team while AI rewrites the rules faster than any of us can track. Not a crisis. Not a pivot. Just: what does operational excellence look like at this scale? Six-plus years in, we've earned that conversation. These moments only exist because of the amazing team we've built, who are building toward the vision David Yadegar, Nona Farahnik Yadegar, and I set out to achieve years ago. I am horrible at remembering to take photos, but our CTO Nick Kavassalis took this of David and me. David is the kind of co-founder who makes you better at the job without making it a thing. Protecting kids from overstimulation. I understand the instinct. I don’t share it. This summer my daughters were in theater camp in Mexico City. Entirely in Spanish. They didn’t sign up for it. They didn’t have a choice. My husband and I worked full time while they navigated a language, a city, and a stage in a country that wasn’t home. Some people hear that and wince. I hear it and think: good. Because here’s what years of doing this has produced — kids who are not afraid of being thrown into something. Who don’t wait to feel ready before they try. Who have learned, through actual experience rather than reassurance, that discomfort is not the same thing as danger. That is not a small thing to give a child. I watch parents work incredibly hard to make sure their kids never feel lost. I think that’s the thing that’s actually making them lost. The world is uncomfortable. It is full of rooms where you don’t speak the language, stages where you don’t know your lines, situations where you have to figure it out in real time. The question isn’t whether your kids will face that. They will. The question is whether they’ll have any practice doing it before it actually matters. My girls were on a stage in Mexico City this summer, performing in a language they were still perfecting, in front of an audience. They also climbed up a blown up hill full of mud. They were more than fine.

Yep, I get it. I'm addicted too. Angry woman summary below: - The Cut piece follows women over 45 spending fortunes on IVF with their own eggs. One did 11 rounds and spent $250,000. No baby. The odds at that age are roughly one in a few hundred. You would not accept those odds anywhere else in your life. - Why do we accept them in fertility? Why does the industry keep selling with odds that low? Fertility is low-risk medicine. You are very unlikely to die from a failed transfer, which sounds reassuring until you realise it means there is almost no consequence for a clinic that keeps selling you one more round. Other areas of medicine have reporting requirements, risk-adjusted scores, regulatory bodies tracking outcomes and consequences if those outcomes are poor. Fertility has the Fertility Clinic Success Rate and Certification Act of 1992 that mandates that all ART clinics report pregnancy success rate data to the CDC. HOWEVER… There is no penalty for non-participation, so most clinics that report only report their best outcome data for younger/easier patients 🫠 - Almost every other field of medicine says "no" when the odds are dismal and the protocol brutal (ok, fine, Plastics and Derm aside). But, oncology does. Transplant does. Neuro does. Fertility says "let's keep going, btw here's the invoice". One REI in The Cut piece said she sees "no harm" in women freezing eggs at 51 because “It’s a free country”. Ehm, what? My take? The science of IVF is truly magic and has built millions of families. The business practice around it is where the rot lives. This is an industry with a hope problem, and it sells hope hard. Hold onto yours. And don't let a fertility clinic sell it back to you.

I had no childcare, a four-month-old on my hip, and a pitch to give that evening in front of a room that could shape the future of my company. I stood in my kitchen and had the argument every working mother knows: Reschedule. You can't show up like this. A real CEO wouldn't walk in with a baby on her hip. Hide it. Apologize for it. I almost did. Instead, I walked into that room with my daughter in my arms, said "my childcare fell through, so she's joining us" — and, heart pounding, I pitched. Here's what I was too scared to see in that moment: She wasn't a liability in that room. She was the entire point. I was asking people to believe in a company built for women exactly like the one I was living out loud — a mother, stretched thin, falling through the cracks of a system never built for her. And I almost hid the very thing that made me the right person to build it. Afterward, a few young women came up to thank me. There's a saying in infant mental health: you cannot be what you cannot see. Because I brought my daughter, they told me they could see it too — that this could be possible for them. I don't lead despite being a mother. A lot of days, I lead because I am one. You don't have to leave her at the door. She might be the most convincing thing you bring into the room. A lot of people close to me have been surprised that I would dive right back into starting a new company — especially another dating company. “Aren’t you tired? Or bored?” The truth is, the deeper I go into the problem of love, the more interesting and inspiring it becomes. Given our rising loneliness epidemic, it’s hard for me to imagine working on anything more meaningful or important. And there has never been a more critical moment. The opportunities and perils of AI are staggering. I worry deeply about its misuse, especially around something as intimate and sacred as love. At the same time, if approached thoughtfully and ethically, I believe we now have the chance to create an entirely new and better paradigm in dating. We'll open in select locations later this year.
